Rewrite perm xlines containing '",' from local opers (to '"\,').

Browse source 
The change also applies to xlines sent to other servers.
Note that the backslash must be supplied in an unxline.
Incoming xlines containing '",' are rejected to avoid
inconsistency between servers.
