* Use NodeFlags to detect nodes in ambient contexts instead of climbing ancestors

* Set context flags on tokens

* Remove 'isDeclarationFile' parameter to 'initializeState' and move to 'parseSourceFileWorker'

* Changes based on code review

* Update API baselines
