Remove references to Pulumi private clouds (#2095)
* Remove TODO for issue since fixed in PPCs.

* Update issue reference to source

* Update comment wording

* Remove --ppc arg of stack init

* Remove PPC references in int. testing fx

* Remove vestigial PPC API types
